CCNX FAQs
The CareConnectNX™ (CCNX) web application gives authorized PMPs and care partners secure access to view the assessment data and care plans of their MDwise Care Select member panel.
This application also allows authorized MDwise staff to document assessments, goals, interventions, status, dates, reviews/evaluations and outcomes. The CareConnectNX tool does not verify eligibility or provide claims information.
How do I receive a user ID and password for the CareConnectNX™ system?
Please sign and fax the CareConnect Acknowledgement Form or call the MDwise Customer Service department at 1-866-440-2449 (or 317-829-8189 if in Indianapolis) and a representative will assist you.
Auto-Assigning Wards of the Court and Foster Children into the Indiana Care Select Program
Per provider banner page BR200817, effective July 1, 2008, all wards of the court and foster children currently enrolled in either Traditional Medicaid or the Hoosier Healthwise Program in the Indiana Health Coverage Programs (IHCP) will be transitioning to the Indiana Care Select Program.
Physicians who have provided primary care services to members who are in these IHCP Programs will be contacted via mail and phone by MDwise Care Select Provider Relations to participate as a PMP in the MDwise Care Select Program so they may continue to care for these patients who continue to need your care and support.
It is important that physicians who are caring for these members carefully consider participation in the MDwise Care Select Program since they know these member’s medical needs the best.
- Increased administrative fee - $15 per member per month (regardless of whether a member is seen).
- After enrollment, PMPs may sign up for CareConnectNX™ (new web based care management tool for PMPs - it's free).
- Access to a care manager who is assigned to the PMPs member (assessed need levels 1 - 4).
- The ability to limit the size/scope of the PMP panel.
- Reimbursement for a semi - annual care coordination conference ($40 per member semi - annually).
- MDwise's 14 years of experience and familiarity with Indiana Medicaid.
