The myMDwise provider portal allows registered providers to view member eligibility information securely online for Hoosier Healthwise, Healthy Indiana Plan and Care Select. Providers enjoy the following online features:

Getting Started
To create a new myMDwise provider portal account, providers must first request an account online. Please fill out form completely and allow 1-5 days for account to be activated.
You will need the following information:

Provider NPI and TIN.

An email address.
Visit the myMDwise provider login page and click on the link which reads "Create a New Account."
Step I
Read and accept the terms of agreement.
Step 2
Enter provider name and address.
Step 3
Enter a contact name, phone, along with practice name and speciality.
Step 4
Enter provider NPI and tax ID number.
Step 5
Enter email address and create your username and password. You will also be asked a "hint" question that is used if you need to reset your password.
Step 6
Verify your information and proceed to site to finish and submit your request.
