Prior Authorization Quick Guide NEW!

Providers should submit PA requests to the care management organization (CMO) that the member is affiliated with on the date of the request.

myMDwise Provider Portal

The myMDwise Provider web portal allows providers to securely view member eligibility for the MDwise Healthy Indiana Plan, Hoosier Healthwise, and Care Select. New users can request an account online. Please fill out form completely and allow 1-5 days for account to be activated. Learn more.
