Due to changes in the Indiana Care Select Program, current MDwise Care Select primary medical providers (PMPs) that wish to continue to participate in the MDwise Care Select PMP network must sign the enclosed MDwise Care Select First Amendment to the Addendum by December 1, 2010.
PMPs employed by a hospital based or independent group/clinic can have an agent of the group or clinic sign the First Amendment on behalf of the group/clinic.

Providers who wish to terminate as a PMP with the new Care Select program must notify MDwise in writing prior to December 1, 2010.
If MDwise has not heard from PMPs prior to December 15, 2010, the Office of Medicaid Policy and Planning (OMPP) and MDwise will assume providers are going to sign the new amendment and plan on continuing as a PMP with Care Select.
PMPs who wish to terminate after December 15, 2010 need to refer to paragraph 62 of the addendum.
