MDwise wants our providers to be informed about the H1N1 vaccine. The newest information we have will be posted for your convenience.
To request participation on the 2009 H1N1 vaccination effort, all public and private facilities must fill out this form and return it to your local health department. Contact information
for each health department is available. The explanation of the H1N1 provider agreement 
is available as well.
Billing Guidance
For reimbursement of administration fee for the H1N1 vaccine this fall, OMPP will be publishing guidelines; refer to Indiana Health Coverage Programs Web site www.indianamedicaid.com
