It is very important to learn about your health care options so that you can make smart choices for your family. MDwise makes it easy and fun to learn.
Members MUST report all changes in your family or income to the State within 10 Days. The link below will take you to the state Report of Change form.

Some examples of changes you must report are:
When someone MOVES IN or MOVES OUT of your home.
When someone in your home gets married, is pregnant, has a baby, or dies.
When someone is covered by health insurance.
When a divorce is final by court order.
When the amount of court-ordered child support you pay changes.
When you move, change a JOB, get a new job, a job ends, an increase or decrease in pay, an employer offers health insurance, or a change in MONEY received such as Child Support or Social Security.
If your family size changes while you are on the Healthy Indiana Plan, you can ask for a recalculation of your contribution at any time.
If you have a job loss or income change you can ask for a recalculation of your contribution once each year that you have HIP coverage. Please call 1.877.GET-HIP9 or 1.877.438.4479 to find out more.

Five Basic Rules
1. Carry your MDwise card with you at all times. Show your card every time you get health care.
2. Contact your doctor for all medical care.
3. Only use the emergency room for true emergencies.
4. You must get preventive health care and checkups.
5. You must pay your monthly contribution to get and stay in this health plan.
