Lost Your Hoosier Healthwise Card?

Everyone on Hoosier Healthwise is assigned a caseworker at the Division of Family Resources (DFR). The caseworker is responsible for sending you cards and replacing lost cards. Call your caseworker right away. If you do not know who your caseworker is, call MDwise Customer Service, 1.800.356.1204 or 317.630.2831 and we can tell you. Your caseworker also determines your eligibility for Hoosier Healthwise.

How Do I Change My Doctor?

If you are a MDwise member, call MDwise Customer Service! We have current lists of doctors that we can mail to you or you can go to the online provider directory to print out a list.

We hope that you will never want to leave the MDwise health plan, but if you do want a doctor outside of our plan, we will make sure you get connected or transferred to the right place to make that change
