MDwise to Reject Claims without a Patient Account Number NEW!
Effective February 1, 2012, the Office of Medicaid Policy and Planning (OMPP) is requiring MDwise to require providers to submit the provider's patient account number on all claims (CMS-1500 and UB-04).

myMDwise Provider Portal
The myMDwise Provider web portal allows providers to securely view member eligibility for the MDwise Healthy Indiana Plan, Hoosier Healthwise, and Care Select. New users can request an account online. Please fill out form completely and allow 1-5 days for account to be activated. Learn more.
