MDwise provides several provider toolkits to assist provider offices and office staff the essential tools and guidelines necessary to achieve a goal or required function. The toolkits are established to help assist in the extra steps needed to learn a new subject rolled out by MDwise or the health plan. The toolkit is designed to provide the tools or educational items to the provider office to simplify or streamline the new process to further assist the office in caring for MDwise members.

40 Weeks of Pregnancy Provider Toolkit

The 40 weeks of pregnancy provider toolkit was a statewide initiative for all four managed care entities to provide education to obstetrician providers on elective inductions and cesarean sections. The toolkit has educational materials for members and provider offices.

Behavioral Health Toolkit

The behavioral health toolkit contains tools for primary medical providers and behavioral health providers that may be useful in their care of MDwise members. The toolkit contains information on ADHD, depression, autism/pervasive developmental disorders, anxiety and ratings scales for use in the diagnosis and treatment monitoring of behavioral health conditions. Additionally, there are materials that can be given to members regarding these diagnoses.

Culturally and Linguistically Appropriate Care Provider Toolkit

The idea of providing culturally competent care versus the practicalities of doing so is an enormous challenge. This toolkit includes several basic tools for providers to implement. The tools and concepts included in the toolkit are those that have been documented as effective in assisting patients from various racial and cultural minority groups as well as those with low health literacy to understand and be understood by their physician.

Diabetes Toolkit for Primary Medical Providers

The diabetes toolkit is designed to help primary medical provider offices achieve quality HEDIS measures in the comprehensive diabetes care measure. The toolkit has diabetic standards for members and tips for provider offices.


School-Based Clinics Toolkit

This provider toolkit is intended to serve as a guide for providers who provide preventive and other medical services to students in their community through a school-based clinic. It is intended to be a tool to provide direction to MDwise providers for submitting claims for covered services.

Tobacco Cessation Toolkit for Providers

Smoking causes an enormous burden on public health. While policy measures to control tobacco use are being applied across the USA, inadequate attention has been given to what health care professionals can do in their routine work with patients. This toolkit is intended to help the health care professional in offering effective interventions for tobacco cessation to patients during office visits.

Quality Provider Toolkit

MDwise is committed to pursuing opportunities for improvement of MDwise Hoosier Healthwise and MDwise Healthy Indiana Plan members’ general health, health outcomes and service through ongoing quality improvement activities and promoting awareness of needed services. This packet includes information for quality improvement activities and education. These materials can help providers understand their quality score, what quality care looks like and how we can work together to make improvements.

Bright Futures Toolkit for Providers

Guidelines for Health Supervision of Infants, Children, and Adolescents is a set of principles, strategies, and tools that are based in theory, driven by evidence, and oriented to systems of care. Bright Futures can be used to improve the health and well-being of all children through culturally appropriate interventions that address the current and emerging health promotion needs at the family, clinical practice, community, health system, and policy levels.
