MDwise wants to extend our warmest greetings and support as you start on this new chapter of your life. We understand that coming back might be challenging and we are here to help you through this transition.

MDwise offers many resources to support you. You can find those resources via HELPlink: gethelp.mdwise.org and searching “re-entry” or visiting the MDwise site. You can also call Customer Service at (800) 356-1204.

Here is a list of the services available

MDwise Resources:

  • Care Management: Nurses will work with you. They’ll help you meet your health care goals.
  • WORKwise: One-stop career center to receive employment and training services in partnership with Midwest Urban Strategies. The program offers: Job and interview training, resume writing, skills tests, education and more. WORKwise can also support those entering the workforce with transportation, childcare and access to uniforms and tools.
  • RIDEwise: Covers rides to and from a doctor, dentist or clinic each year. If available in member’s area, MDwise may give bus passes for trips to the doctor. Members can also request gas mileage reimbursement.
  • RECOVERYwise: Initial financial support to move into a recovery house – referrals can be completed at RECOVERYwise.

  • Trusted Mentors: Connect members to mentorship programs and Opioid Use Disorder services
  • Groups Recover: Opioid use disorder support
  • Edna Martin Center: Wrap around services
    • Housing support
    • Education – Two-week job readiness training, stipends when attend trainings
    • Career – Job readiness (Construction, CNA, IT, Pharmacy Tech), employer connections and resumes

    • Employment
    • Re-entry
