Steps for New Members

Welcome to MDwise!

MDwise has got you covered! As your partner in healthcare, our coverage goes beyond the cost of health services. Whether you need a ride to an appointment or help quitting tobacco, we’re here to come alongside you and make taking care of your family’s health a little bit easier. As a nonprofit, local healthcare provider, we’ve been taking care of Indiana families since 1994.

MDwise provides high-quality, affordable care for Hoosiers through two Medicaid plans: Healthy Indiana Plan and Hoosier Healthwise. Once enrolled in one of these plans, you can begin taking advantage of the great coverage and programs MDwise offers by completing a few simple steps.

Create a Portal Account

To get started, create an account in MDwise's online member portal, myMDwise. The portal is secure, confidential and easy to use. With the myMDwise member portal, you can:

  • View the name of your current doctor or request a change
  • View the status of medical and pharmacy claims
  • View or print your member ID card
  • View MDwise REWARDS points and get gift cards
  • And more!

To create an account, you'll need your member identification MDwise (MID) number, which you can find in your mailed welcome packet or by calling customer service at 877-822-7193. You can access the member portal 24 hours a day, seven days a week.

Complete Your Health Survey

Next, you'll need to complete a quick (we promise!) health survey. The survey gives MDwise a better understanding of your health needs so that we can provide better, more personalized care.

Take your health survey within 90 days of becoming a member to earn a $30 Visa gift card. If multiple people in your household are new MDwise members, you can complete the survey for each family member and receive a gift card for each completed survey!

Two ways to take the survey:

You can expect a call from the MDwise care team within two weeks of taking the survey. We will answer any questions you have and help connect you to doctors and services.

Get Connected with a Doctor

The third step is making an appointment with a primary care physician or an OB if you’re pregnant. Give MDwise a call so we can make sure we have the right doctor on file for you. We can also set you up with one should you need assistance. Regularly scheduling physicals and preventative care visits is one of the best ways you can live your healthiest life.

Remember, all (yes, all!) doctor visits are FREE with HIP and Hoosier Healthwise!

Manage Your Conditions

MDwise wants to help you stay as healthy as possible. That’s why we have nurses and clinical staff (also called care managers) on our team to help you manage your individual health needs.

Our INcontrol program can be especially helpful for members with certain health conditions like diabetes, asthma, ADHD or depression.
